Initial inquiries
The review preview does not transmit consultation or referral form data. Telephone and email links allow you to contact Vital Path Care directly. Please do not send medical records, diagnoses, health card numbers, claim documents or another person’s confidential information through a general website inquiry or ordinary email.
Information and purpose
When the inquiry form is activated, its intended fields are name, contact details, municipality, broad service category and inquiry type. These details are used to respond to the inquiry and discuss suitable next steps, not to provide a medical assessment or guarantee care. The submission provider and retention period must be published here before activation.
Hosting and third parties
Website hosting providers may process technical request information such as IP addresses and access logs. The preview is hosted through ChatGPT Sites with private access. The intended public website host is Cloudflare Pages. External services, including email and linked application forms, have their own privacy practices. The careers, consultation and referral forms are built into this website. In the review preview they do not transmit or save submissions. No Jotform application is embedded or used by these forms. Email delivery must be connected and tested before activation.
Analytics and your choices
Google Analytics is not active in this preview. If enabled on the public site, optional analytics must be explained and consent obtained through the configured preference control. Names, phone numbers, email addresses, form values and health details must not be sent to analytics.
Career applications
The career application requests contact details, role, work eligibility, driving and vehicle access, experience, availability, certification information, vulnerable-sector-check status, a required résumé and optional certification uploads. The prepared setup stores application records and files in private storage, then sends a reference notification to our recruitment email through a separately configured email delivery service. The preview does not send or save applications or files. The email processor, retention practices and authorized recipients must be confirmed before activation. Do not upload identity documents, a SIN, banking details, medical records or police-check reports at this initial stage. Contact careers@vitalpathcare.ca with application privacy or deletion questions. Recruitment access permissions and retention practices must be reviewed before public launch.
Safeguards and detailed intake
Detailed care information should be collected through a separately agreed intake process with appropriate consent and access controls. Avoid submitting someone else’s personal information without authorization.

Changes before launch
This preview policy must be finalized to reflect the chosen form processor, retention schedule, analytics settings and actual business privacy procedures before the replacement goes live.
